Job summary

American River Home Care is hiring an Evening Intake, Scheduling specialist to connect our office, patients, and field clinicians in a home health environment. You will manage referrals, verify coverage, and ensure accurate EMR data entry to support timely patient care.

This role requires strong multitasking, excellent communication with families and providers, and comfort with EMR and phone systems. Weekend rotation is required; schedule includes 3:30 PM–12:00 AM shifts, with days off Thursday

Qualifications

• Prior healthcare experience preferred (home health, hospice, clinic, or hospital).
• Strong multitasking, organizational, and problem-solving abilities.
• Experience with scheduling in a healthcare or service setting is a plus.
• Clear, professional communication with patients, families, and providers.
• Comfortable with technology (EMR, email, phone systems).

Responsibilities

• Intake and Scheduling coordination between office, patients, and field staff.
• Process new referrals quickly and accurately.
• Verify insurance and collect necessary documentation; enter data into EMR.
• Coordinate and schedule patient visits with clinical staff based on needs.
• Communicate with hospitals, clinics, and physicians to obtain missing information.
• Adjust schedules for PRN needs, cancellations, or urgent requests.
• Provide backup coverage for other administrative or field support roles.

Evening Intake, Scheduling – Home Health

**Job Summary:**
Be the Link Between Our Office, Patients, and the Field. American River Home Care is seeking a dependable, organized, and detail-oriented team member for a role combining patient intake and scheduling duties. You'll coordinate care, manage schedules, and ensure patients and clinicians have the resources they need when they need them. If you have healthcare experience, thrive on multitasking, and enjoy variety in your workday, this is the role for you.

**Schedule:**
Saturday, Sunday, Monday, Tuesday, Wednesday 3:30pm-12:00am with one weekend off per month.
Days off on Thursday and Friday

**Responsibilities:

• Intake & Scheduling
• Process new referrals quickly and accurately.
• Verify insurance, collect necessary documentation, and enter patient data into EMR.
• Coordinate and schedule patient visits with clinical staff based on patient needs, clinician availability, and route efficiency.
• Communicate with hospitals, clinics, and physician offices to obtain missing information or orders.
• Adjust schedules for PRN needs, cancellations, or urgent visit requests.
• General
• Anticipate and respond to care team needs to prevent delays in service.
• Provide backup coverage for other administrative or field support roles when needed.

**Qualifications:

• Prior healthcare experience (home health, hospice, clinic, or hospital preferred).
• Strong multitasking, organizational, and problem-solving skills.
• Experience with scheduling in a healthcare or service setting is a plus.
• Clear, professional communication with patients, families, and healthcare providers.
• Comfortable with technology (EMR, email, phone systems).
• Ability to lift/carry up to ~25 lbs.
• Flexibility to work rotating weekends and cover any of the available shifts.
