Job summary

Beth Israel Lahey Health at Home in Woburn seeks an Evening Hospice RN to provide patient-centered care two evenings per week from 4:00 pm to 9:00 pm. The role emphasizes assessment, planning, and symptom management in the home hospice setting.

You will complete initial assessments, develop patient-specific care plans, educate families about medications and the dying process, supervise Hospice Aides, and participate in bi-weekly hospice conferences.

Qualifications

• Active Massachusetts RN license required.
• Associate’s Degree or BSN preferred.
• BLS certification required or equivalent.

Responsibilities

• Complete and document an initial assessment of the patient and family to determine hospice care needs.
• Perform a complete physical assessment of the patient and history of current and previous illness(es).
• Provide information to the patient and family regarding the dying process and expectations.
• Develop a patient-specific plan of care with goals based on nursing diagnosis and pain/symptom management.
• Provide instruction to patient and family regarding medication actions, side effects, dosage, route, and frequency; supervise Hospice Aides; participate in case conferences.
