Job summary

Beverly, MA-based Beth Israel Lahey Health at Home is hiring an Evening Hospice RN (1:00pm–9:00pm) for full-time 40 hours/week. You will plan and deliver at-home hospice care using the nursing process with patients, families, and the interdisciplinary team.

The ideal candidate has an Associate’s Degree or BSN, a MA RN license, BLS, and at least 1 year of direct care experience (home care/public health preferred). Salary range posted: $83,200–$129,626 annually.

Qualifications

• Associate’s Degree or BSN from an accredited nursing program.
• Massachusetts RN license active.
• BLS certification active.
• At least 1 year direct care nursing experience (home care/public health preferred).
• EPIC experience preferred.

Responsibilities

• Complete and document an initial assessment of patient and family to determine hospice care needs.
• Perform a complete physical assessment of patient and history of current and previous illnesses.
• Provide information to patient and family regarding the dying process and expectations.
• Develop a patient-specific plan of care with goals based on nursing diagnosis and therapeutic, preventive, and palliative actions.
• Provide instruction to patient and family regarding medication actions, side effects, dosage, route, and frequency; document services using problem-oriented records.
• Instruct and supervise care by Hospice Aides; supervise every two weeks minimum and as needed.
• Participate in bi-weekly hospice case conferences.
