• Perform Start of Care (SOC) assessments.
• Resumption of Care visits and PRN visits.
• Respond to after-hours calls and triage concerns.
• Coordinate care with case managers and interdisciplinary teams.
• Provide wound care and disease management education.
• Monitor urinary issues and post-surgical urology follow-up.
