• Active Florida RN license or Compact RN license.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete SOC admissions and skilled nursing revisits.
• Residency ROC visits and comprehensive patient assessments.
• Develop and update individualized care plans and wound care.
• Manage IV starts and disease management education.
• Coordinate care with physicians and interdisciplinary teams.
• Complete OASIS documentation and EMR charting.
• Participate in rotating evening/weekend on-call coverage.

What You'll Do

Provide skilled nursing care to patients in their homes while ensuring continuity of care after normal business hours.

Responsibilities include:

• Perform comprehensive patient assessments
• Develop and update individualized care plans
• Wound care
• Disease management education
• IV starts
• Monitor urinary retentionPost-surgical urology care
• Coordinate care with physicians and interdisciplinary teams
• Complete OASIS documentation and EMR charting
• Participate in rotating evening/weekend on-call coverage

What We're Looking For

Required:
