Job summary

Concierge Home Care is seeking an experienced Registered Nurse (RN) to provide skilled evening home health visits throughout Clay County, FL. This after-hours role offers Mon-Fri 4:00 PM–10:00 PM with no overnight shifts and an attractive per-visit compensation plus an evening differential.

You'll perform SOC admissions, revisits, and complex care plans, using AI-assisted charting and WellSky EMR, while coordinating with physicians and the clinical team to ensure safe, high-quality home care.

Qualifications

• Active Florida RN License or Compact RN License required.
• Valid driver's license required.
• Reliable transportation required to perform home visits.

Responsibilities

• Provide skilled nursing care to patients in their homes after hours.
• Complete Start of Care (SOC) admissions.
• Skilled nursing revisits and Resumption of Care visits.
• Develop and update individualized care plans.
• Wound care and IV starts as needed.
• Coordinate care with physicians and interdisciplinary teams.
• Complete OASIS documentation and EMR charting.
• Participate in rotating evening/weekend on-call coverage.

Schedule

• Urgent nursing visits (Up to 10:00 PM)

What You'll Do

Provide skilled nursing care to patients in their homes while ensuring continuity of care after normal business hours.

Responsibilities include:

• Perform comprehensive patient assessments
• Develop and update individualized care plans
• Wound care
• Disease management education
• IV starts
• Monitor urinary retention
• Post-surgical urology care
• Coordinate care with physicians and interdisciplinary teams
• Complete OASIS documentation and EMR charting
• Participate in rotating evening/weekend on-call coverage

  What We're Looking For
