Concierge Home Care in Jacksonville, FL is seeking a detail-oriented Home Health Registered Nurse (RN) for after-hours coverage.

This part-time/modified full-time role offers evening shifts, a flexible schedule, and per-visit compensation with an evening differential. You will perform SOC and Resumption of Care visits, triage after-hours calls, coordinate with case managers, manage wounds and urology care, and document promptly in WellSky/Kinnser EMR.

Qualifications

• Active Florida RN license or Compact RN license.
• Home Health experience preferred; urology, Med-Surg, Hospice or Acute Care backgrounds valued.
• Strong assessment and triage skills with independent work style.
• Ability to manage after-hours coverage and urgent patient needs.

Responsibilities

• Complete skilled nursing visits during weekday evenings.
• Start of Care and Resumption of Care visits; PRN visits as needed.
• Respond to after-hours patient calls and triage concerns.
• Coordinate care with case managers and interdisciplinary teams.
• Provide wound care and disease management education.
• Support urinary-related patient needs and post-surgical follow-ups.
• Document promptly within EMR timeframes.

  Position Responsibilities

  The After-Hours RN provides clinical support outside of traditional business hours and helps ensure continuity of care while reducing unnecessary hospitalizations.

  Responsibilities include

  • Complete skilled nursing visits during weekday evenings
  • Perform:
  • Start of Care (SOC) assessments
  • Resumption of Care visits
  • PRN visits

  Respond to after-hours patient calls and triage concerns appropriately

  Coordinate care with:

  • Case managers
  • Interdisciplinary care teams

  Provide skilled clinical care including:

  • Wound care
  • Disease management education

  Support urology-related patient needs including

  • Urinary retention monitoring
  • Post-surgical urology follow-up

  Complete documentation within required EMR timeframes

  Schedule Structure

  Monday – Friday

  On-call availability: 3-4:00 PM – 9:00 PM

  Coverage may include:

  • Start of Care admissions
  • Late-add visits
  • Telephone triage support

  Productivity Expectations

  Minimum weekly productivity: 18 points
