Job summary

Concierge Home Care in Orange Park, FL is seeking a Home Health Registered Nurse (RN) for after-hours shifts. The role focuses on independent clinical care outside standard hours, ensuring continuity of care and reducing unnecessary hospitalizations.

Responsibilities include evening/weekend visits, SOC assessments, and collaboration with care teams. Flexible schedule and a supportive clinical environment offered.

Qualifications

• Active Florida RN license or Compact RN license required.
• Preferred Home Health experience and familiarity with urology, Med-Surg, Hospice or Acute Care.
• Strong assessment and triage skills; able to work independently.

Responsibilities

• Provide skilled nursing visits during evenings and weekends (After-Hours).
• Complete SOC, Resumption of Care, and PRN visits.
• Coordinate care with case managers and interdisciplinary teams.
