Job summary

Qualifications

• High School diploma or GED is required.
• BLS certification is required.
• Current Ohio LPN license; must be in good standing with the Ohio Board of Nursing and have completed an Ohio Board of Nursing approved medication course.

Responsibilities

• 25% Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual.
• 25% Documents planned nursing interventions based on nursing diagnoses and collaborative problems; initiates and coordinates care plans.
• 20% Evaluates and documents response to nursing interventions and achievement of outcomes; revises plan of care as needed.
• 20% Delegates tasks per criteria and state statutes; reviews work performed by those to whom it is delegated; coordinates continuity of care.
• 10% Participates in process improvement, patient advocacy, staff meetings, and collaboration with providers; manages resources to maintain a safe, efficient environment.
