Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with ability to transport individuals in agency vehicles.
• Ability to complete required background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment.
• Commitment to continuity of care, including occasional stay beyond scheduled shift.

Responsibilities

• Support individuals in daily living activities and personal care.
• Monitor health information and document progress in the EHR.
• Assist with medications and maintain accurate medication records.
• Transport individuals to appointments and activities using agency vehicles.
• Provide nutrition assistance and support during meals.
• Assist with mobility, transfers, and safety.
