Avamere Three Fountains in Medford, OR is seeking a Certified Nursing Assistant (CNA) for the full-time Evening shift (2:00 PM to 10:00 PM). You will provide routine daily nursing care to residents according to care plans and assessments.

Qualifications

• Must hold an active Oregon CNA license.
• CPR/BLS certification required.
• High school diploma or equivalent.

Responsibilities

• Provide routine daily nursing care per resident care plans.
• Check residents and ensure personal care needs are met.
• Review and update care plans; inform nurse supervisor of changes.
• Assist with daily activities and transportation to appointments.
• Create a warm, calm environment and communicate resident information to the team.
• Prepare rooms for residents and participate in surveys as required.
