Job summary

Avamere is seeking a Certified Nursing Assistant (CNA) for the Evening shift (2:00 PM to 10:00 PM) at Avamere Three Fountains, 835 Crater Lake Ave., Medford, OR. The role focuses on delivering routine daily nursing care per each resident's assessment and care plan.

The primary duties include supporting residents with daily needs, reviewing care plans, and communicating changes to the nursing team, all while fostering a warm, patient, and cheerful environment.

Qualifications

• Requires a valid CNA license in the state of Oregon.
• CPR/BLS certification must be active during employment.
• Must have a passion for caregiving and improving residents' quality of life.

Responsibilities

• Provide routine daily nursing care per resident care plan.
• Review care plans daily and note changes.
• Inform nurse supervisor of any changes in resident condition.
• Assist with daily activities and transportation to appointments.
• Maintain a warm, patient, and cheerful environment.
• Prepare rooms and support residents to feel comfortable.
• Participate in facility surveys and communicate resident information to nurses/administration.
