Job summary

Acloché LLC is seeking an Admissions Nurse (RN) in Connecticut to oversee evening clinical intake for new residents. You will coordinate with hospital discharge planners, physicians, leadership, and families to ensure a smooth transition into the center, focusing on initial assessments, medication reconciliation, and rapid chart setup.

The role emphasizes evening admissions (3:00 PM – 11:00 PM) with a direct-hire, on-site arrangement.

Qualifications

• Active CT RN license required.
• 1–2 years in SNF/LTC or sub-acute setting preferred.
• Admissions, discharge coordination, or desk nursing desirable.
• EMR proficiency with PointClickCare for rapid chart initiation.
• Strong organizational and communication skills with families and hospital staff.

Responsibilities

• Lead evening intake and onboarding during the 3–11 shift.
• Perform initial nursing assessments and admission documentation.
• Reconcile hospital summaries and orders for immediate med continuity.
• Coordinate with discharge planners and care teams for room setup.
• Develop preliminary care plans and brief on-duty supervisors.
• Orient residents and families to policies and routines.
