Job summary

CareerNerds, LLC is seeking an Admissions Nurse (RN) in Connecticut to oversee evening clinical intake and streamline admissions. You will coordinate with discharge planners, physicians, staff, and families to ensure smooth transitions and accurate charting.

The role emphasizes initial assessments, medication reconciliation, and opening electronic charts, enabling floor staff to focus on direct resident care. Licensed CT RN with 1–2 years in SNF/LTC is ideal.

Qualifications

• Active, unrestricted RN license in Connecticut.
• 1–2 years nursing experience in SNF/LTC/sub-acute rehab preferred.
• Experience with admissions, discharge coordination, or unit desk nursing is desirable.
• Hands-on EMR experience for rapid chart initiation.

Responsibilities

• Lead evening intake and onboarding for admissions (3:00 PM–11:00 PM).
• Perform initial nursing assessments and admission documentation.
• Reconcile hospital discharge summaries and physician orders for continuity.
• Coordinate with discharge planners, care teams, and families for room setups and equipment.
• Initiate preliminary care plans and communicate needs to on-duty supervisors.
• Orient new residents and families to policies and routines.
