Responsibilities

• Assessment: admission and ongoing assessment of patient condition and care needs.
• Planning: design and document plan of care and discharge goals.
• Implementation: prepare patients for treatments and report changes to team/physician.
• Evaluation: reassess and adjust care plans as needed.
• Report Skills: provide bedside shift reports for continuity of care.
• Environment & Patient Safety: maintain safe and clean patient environment and report hazards.
