Job summary

Responsibilities

• Perform initial and ongoing patient assessments and care plans.
• Design, direct, evaluate, and document plans of care with patient/family teaching and transitions of care.
• Prepare patients for treatments, surgeries, and diagnostic studies; report changes to the physician.
• Reassess patients and adjust care based on changing needs.
• Maintain bedside shift reports to ensure continuity of care.
