Qualifications

Responsibilities

• Assess patients’ physical, psychosocial, cultural, and spiritual needs.
• Evaluate patient responses and intervene to achieve optimal outcomes.
• Develop and implement a plan of care with the interdisciplinary team.
• Document patient status, interventions, and outcomes accurately.
• Use IT tools for communication, knowledge management, and decision support.
• Provide patient and family education and assess teaching effectiveness.
