Job summary

UMass Memorial Health - Milford Addiction Care is seeking a Recovery Support Navigator to provide care management and navigation supports for patients with substance use disorders and co-occurring mental health conditions. You will engage patients, assist in accessing treatment and community resources, and participate as part of the care team.

The role emphasizes education, care coordination, and helping patients set personal goals for recovery within an interprofessional team.

Qualifications

• High school diploma or equivalent.
• Valid driver's license.
• Excellent communication and relationship-building with patients, teams, and healthcare professionals.
• Willingness to work flexible hours, including evenings and weekends.
• Fluent written and spoken English.
• Proficiency with MS Word and Excel for documentation.
• Knowledge about treatment and recovery from substance use disorders.

Responsibilities

• Develop goals and objectives with the patient based on identified needs.
• Determine whether a comprehensive care plan exists and coordinate with providers.
• Connect patients with providers to develop a care plan if needed.
• Support patients in understanding treatment options, including FDA-approved pharmacotherapy.
• Provide information about community and recovery supports and family resources.
