Job summary

UMass Memorial Health in Central Massachusetts is hiring a Point of Entry/Emergency Department Case Manager (POE/ED CM) to coordinate care for ED patients, assess needs, and determine appropriate discharge dispositions in collaboration with the ED and admitting teams.

The role requires a BSN and Massachusetts RN license with a minimum of 3 years in medical/surgical/ICU/ED nursing; CCM certification is preferred. A signing bonus may be available and internal equity will be considered.

Qualifications

• Bachelor of Science in Nursing and MA RN license required.
• Minimum 3 years of medical, surgical, ICU, or ED nursing experience.
• CCM certification preferred.

Responsibilities

• Coordinate and facilitate ED patient care management.
• Assess patient needs and communicate discharge options (Home, Post-acute, Inpatient, or Observation).
• Complete admission reviews and ensure medical necessity with care team.
• Plan discharge and arrange post-acute services (Home Health, DME, SNF, rehab, medications).
• Escalate unresolved cases and document delays as needed.
• Demonstrate age-appropriate communication and care for all patients.
