Job summary

Qualifications

• Three (3) years clinical experience in discharge planning or case management.
• Ability to use specialized computer-based systems for gathering, reporting, and analyzing clinical data.
• Excellent organizational and communication skills.

Responsibilities

• Assess patient’s physical, functional, and psychosocial needs.
• Perform admission and concurrent reviews and communicate with third‑party payers.
• Coordinate care across the continuum with care coordination team.
• Collaborate with physicians, care providers, and community resources in care planning.
• Contact third party payer to clarify benefit eligibility and resources.
• Monitor and evaluate discharge plan outcomes with patients and family.
• Educate patients, families, and team on care decisions and resources.
• Engage in quality improvement and compliance with standards.
• Support department and center-wide performance improvement programs.
