Qualifications

Responsibilities

• Coordinate with the care team to promote health and wellness.
• Communicate patient data to the care team in a timely manner.
• Follow infection prevention and standard precautions in all tasks.
• Assist with activities of daily living and record vitals and weight.
• Provide care guided by compassionate patient-centered practice.
