Job summary

Fresenius Medical Care seeks an entry-level MSW to provide psychosocial services to nephrology patients, including in-center and home dialysis when applicable. You will assess, counsel, and support patients and families, collaborate with the care team, and address barriers to adherence and quality of life.

Responsibilities include care planning, patient education, referrals to resources, and participation in Quality Improvement activities to support FMCNA goals.

Qualifications

• Masters in Social Work required.
• Must have state temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW license (if required by state) and obtain licensure to work independently within the first two years.

Responsibilities

• Assess patients' psychosocial status and needs as part of comprehensive patient assessment.
• Participate in care planning with patient and healthcare team to identify effective interventions.
• Provide counseling and referral to community resources to support adherence and quality of life.
• Educate patients and families about transplant options and advance directives.
• Document interactions and contribute to quality improvement activities and patient education.
