Job summary

Fresenius Medical Care in the United States is hiring an entry-level Medical Social Worker (MSW) to support psychosocial services for in-center and home dialysis patients. The role works with an interdisciplinary team to promote adjustment, rehabilitation and quality of life, while educating staff on the impact of chronic kidney disease on patients and families.

Qualifications

MUST have a Master of Social Work degree.
Must hold or obtain an appropriate state social work license.
Obtains licensure to practice independently within the required state timeframe.

Responsibilities

Assess patients’ psychosocial status, strengths and needs as part of comprehensive assessment.
Collaborate with interdisciplinary team to plan interventions improving rehabilitation and quality of life.
Provide counseling and education to patients and families regarding psychosocial issues of ESRD.
Refer to community resources and coordinate with home health or vocational services as needed.
Support patients in understanding advance directives and DNR discussions in collaboration with the team.
Document MSW interactions and interventions per policy and regulatory requirements.
Engage in quality improvement activities and participate in team huddles.
