Job summary

Fresenius Medical Care is seeking an entry-level Medical Social Worker (MSW) to support patients undergoing dialysis, both in-center and home-based care where applicable. You will collaborate with the interdisciplinary team to assess psychosocial needs, promote rehabilitation and improve quality of life, while guiding patients through transplant referrals and community resources.

The role requires a Master’s in Social Work and state licensure, with 0–2 years of related experience.

Qualifications

• Master's in Social Work required and state license.
• 0–2 years of related social work experience.
• Meets applicable scope of practice and licensure requirements in the state employed.

Responsibilities

• Assess patients' psychosocial status and care planning in collaboration with the healthcare team.
• Provide counseling to patients and families and support transplant referrals.
• Educate patients on resources, advance directives, and community services.
• Support quality improvement activities and adherence to unit policies.
