Job summary

Fresenius Medical Care is seeking an entry level Medical Social Worker to provide psychosocial services for in-center and home dialysis patients as part of the interdisciplinary team. You will assess needs, educate patients and families, and support treatment adherence to improve quality of life.

The role emphasizes collaboration with clinicians, care planning, and connections to community resources. A state license and MSW degree are required, with 0–2 years of related experience expected.

Qualifications

• Masters in Social Work required.
• Must have state required license.
• Meets the applicable scope of practice board and licensure requirements.
• 0 – 2 years’ related experience.

Responsibilities

• Assess patients' psychosocial status and participate in care planning with the interdisciplinary team.
• Provide counseling and educational support to patients and families to enhance adjustment and quality of life.
• Refer and connect patients to community resources (home health, vocational rehab, etc.) to aid treatment goals.
• Support transplant referrals and manage waitlist coordination with the team.
• Participate in monthly quality reviews and contribute to quality indicators.
