Job summary

Qualifications

• Masters in Social Work required.
• Must have state required license.
• Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.

Responsibilities

• Assess patients' psychosocial status, strengths and needs as part of the comprehensive patient assessment.
• Participate in care planning with the patient and healthcare team to identify interventions to meet rehabilitation, treatment goals and quality of life improvements.
• Provide monitoring and interventions to help patients adjust to dialysis and achieve psychosocial well-being.
• Provide supportive counseling to patients and families as permitted within scope of training and licensure.
• Assist with transplant referrals and educate patients about psychosocial aspects of ESRD.
