Job summary

Qualifications

• Masters in Social Work required.
• Must have state-required license.
• Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed.

Responsibilities

• Assess patients' psychosocial status and develop care plans.
• Provide counseling and educational services to patients and families.
• Collaborate with interdisciplinary team to address barriers and improve quality of life.
• Assist with transplant referral process and waitlist management.
• Coordinate with insurance coordinators on financial assistance.
