The role requires a High School Diploma or equivalent, with preferredAssociate's degree and Recovery Support Specialist or CHW training; bilingual Spanish/English is a plus. A valid driver’s license is required.

Qualifications

• High School Diploma or equivalent required.
• Associate's degree preferred.
• Recovery Support Specialist certification or CHW education preferred.
• Bilingual in Spanish and English preferred.
• Valid driver’s license and vehicle insurance.

Responsibilities

• Support patients on the mobile unit in Springfield, including initial assessments and transportation.
• Deliver medications on the mobile unit to patients enrolled in treatment.
• Conduct outreach to people who use drugs, those experiencing homelessness, and community members.
• Oversee services on FGC’s Mobile Van and collect GPRA data.
• Provide customer service and document in patient records per IL.DASA and FGC standards.
• Demonstrate professionalism when interacting with patients, staff, and partners; participate in supervision and continuing education.
• Describe FGC programs and assist access to treatment.
