Job summary

The Center for Human Development (CHD) is seeking a Community Support Program Worker for our Springfield outpatient clinic. You will provide outreach and linkages to the community, coordinate services, and support clients to achieve their treatment goals across outpatient and other care settings.

Responsibilities include delivering mobile outreach, aiding with benefits, housing and health care access, and participating in crisis planning and safety measures.

Qualifications

• Bachelor’s degree or at least three years of relevant work or lived experience.
• Ability to pass certification for CPR, First Aid.
• Valid driver’s license and reliable transportation for business use.

Responsibilities

• Provide service coordination and linkages to the community.
• Provide outreach services delivering mobile services in community or institutional settings.
• Assist clients in obtaining benefits, housing and health care as needed.
• Provide crisis support and participate in crisis prevention and safety planning.
• Encourage use of natural support systems and recovery-oriented services.
