Responsibilities

• Support patients on the mobile unit in Springfield, including initial assessments and transportation.
• Deliver medications on the mobile unit to patients enrolled in treatment.
• Conduct outreach to people who use drugs, those experiencing homelessness, and community members.
• Oversee services on FGC’s Mobile Van and collect GPRA data.
• Provide customer service and document in patient records per IL.DASA and FGC standards.
• Demonstrate professionalism when interacting with patients, staff, and partners; participate in supervision and continuing education.
• Describe FGC programs and assist access to treatment.

Family Guidance Centers, Inc. (FGC), a not-for-profitbehavioral healthcare organization, treats and prevents substanceabuse, as well as an array of other related problems. • Supporting patients accessing care on FGC’s mobile medication unitin [Springfield], including conducting initial assessments, arrangingtransportation, and ensuring ongoing engagement in treatment at acommunity-based provider.
• Delivering medications on the mobile unit to patients alreadyenrolled in treatment services.
• Conducting outreach to people who use drugs, individualsexperiencing homelessness, and community members.
• Overseeing and supporting services delivered on FGC’s Mobile Van,including completing the Government Performance Results Act (GPRA)data collection instrument.
• Providing positive customer service that promotes quality patientoutcomes, and documenting all services in the individual patientrecord in a timely manner and in accordance with IL.DASA, Federal, andFGC standards.
• Demonstrating professionalism when interacting with patients,staff, outside professionals, and collaterals; participating insupervision with the Program Manager and engaging in continuingeducation activities related to care management and maintenance ofrelevant certification or licensure.
• Describing FGC programs, services, and eligibility requirements inclear, accessible language to individuals struggling with opioid oralcohol addiction, those at risk of overdose, and their family membersor friends, and assisting them in obtaining immediate access to treatment.

Schedule: Must work a minimum of 40 hours per week, Monday-Friday 7am-3:30pm. Hours may vary based on program needs of the agency and flexibility is required.

Pay Rate : $20 - $21/hr.

Experience and Skills:

Education and/or Experience: High School Diploma orequivalent. Associate's degree preferred. Experience in behavioralhealth, community health outreach, substance use harm reduction,withdrawal management programs, medication-assisted treatment,and/or community relations required. Certification as a RecoverySupport Specialist or education as a Community Health Workerpreferred. Bilingual in Spanish and English preferred.
