Job summary

Centerstone seeks a Navigator to maximize enrollment in comprehensive insurance coverage and to build community partnerships for outreach activities. The role focuses on helping individuals and small businesses with eligibility options and guiding them through enrollment, while engaging with healthcare and social service systems.

The Navigator will work to empower patients, reduce barriers to access, and represent Centerstone in community health initiatives.

Qualifications

• High school diploma or GED required.
• Bachelor’s degree in Social Work, Psychology, Public Health, or related field preferred.
• 2 years of experience in community health, human services outreach, or healthcare preferred.

Responsibilities

• Help individuals with insurance eligibility options.
• Help small business employers and their employees with insurance eligibility options.
• Facilitate insurance enrollment.
• Provide insurance information in the best interest of the consumer.
• Facilitate communication with healthcare/social service systems.
