Qualifications

• High School Diploma or equivalent required.
• Two years in healthcare outreach, case management, or related field preferred.
• Knowledge of Medicaid/Medicare enrollment helpful.

Responsibilities

• Conduct outreach to individuals eligible for managed care and healthcare programs.
• Build trusting relationships with patients through phone, virtual, and in-person efforts.
• Educate on available services, preventative programs, and wellness resources.
• Assist with enrollment, eligibility determinations, and plan selection.
• Coordinate with providers, plans, and community partners to remove barriers to care.
