Job summary

Trinity Health is seeking an organized Inquiry & Intake Support Specialist for Eddy Senior Care PACE. This role serves as the first point of contact for prospective participants and families, screens inquiries for basic eligibility, and routes qualified leads to Intake/Enrollment Specialists.

You will assist with data entry, packet preparation, and event support for the Community Liaison team and PACE program, contributing to a collaborative, mission-driven environment.

Responsibilities

• Serve as the initial contact for incoming inquiries (phone, message, referral).
• Gather key information to help determine basic eligibility and level of interest.
• Document and track all inquiries (qualified and non-qualified) accurately and timely.
• Route qualified leads to the appropriate Intake/Enrollment Specialist for follow-up.
• Provide clear, professional information about the PACE program to callers and families.
• Maintain consistent communication and a positive experience for all prospective participants.
