Job summary

Open Sky Community Services is seeking an Enrollment and Care Coordination Specialist in Massachusetts. You will screen individuals for CSP-HI, housing, nutrition, and related eligibility and enroll them in the program, including orientation to services and data entry into the electronic health record system.

You will train and monitor housing specialists, coordinate with the team lead, and help manage caseloads to ensure smooth service delivery for community members in need.

Qualifications

• High School Diploma or GED required.
• At least 2 years’ experience with housing and behavioral health populations.
• Valid driver’s license and acceptable driving history.
• Collaborate with a multi-disciplinary team with excellent written and verbal communication and attention to detail.
• Experience with electronic health records, billing, or referral management is preferred.

Responsibilities

• Screen individuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility.
• Complete enrollment for individuals including orientation, consents, and data entry into the EHR.
• Provide training, monitoring and corrective action to housing specialists and track caseloads with the team lead.
