Open Sky Community Services is seeking an Enrollment and Care Coordination Specialist in Massachusetts. You will screen individuals for CSP-HI, housing, nutrition, and related eligibility and enroll them in the program, including orientation to services and data entry into the electronic health record system.
You will train and monitor housing specialists, coordinate with the team lead, and help manage caseloads to ensure smooth service delivery for community members in need.
Job Description: The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection.
The Enrollment and Care Coordination Specialist will screen individuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboration with the team lead, as needed.
