Job summary

Open Sky Community Services in Massachusetts is seeking a Community Supports Navigator to provide case management and care coordination for individuals with mental health disorders at high risk for admission to 24-hour treatment settings.

You will develop a Support Plan, complete assessments, serve as liaison among members, families and community providers, assist with transportation to appointments, participate in on-call rotations, and perform outreach as needed.

Qualifications

  • High School Diploma or GED required.
  • Driver’s license with reliable transportation required.
  • Minimum 2 years’ experience with homelessness, justice involvement, adults with serious mental illness, substance use disorders, or underserved populations.

Responsibilities

  • Develop a Support Plan for each member and guide services accordingly.
  • Complete Assessments and Support Plans within prescribed timelines.
  • Act as liaison between members, families, and community agencies to coordinate treatment.
  • Collaborate with medical/behavioral health providers to align care plans.
  • Provide temporary transportation assistance to appointments as needed.
  • Participate in an On-Call rotation.
  • Perform outreach duties as needed.

Job Description:Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting.The Community Services Navigator will provide case management and direct outreach support to their assigned caseloads, which consist of members enrolled in Community Services Programs, Recovery Support Navigation, Behavioral Health Justice Involved and/or Community Support Program for People Experiencing Chronic Homelessness.Other Key Responsibilities:Develop a Support Plan with the member that guides services in a way that is consistent and supportive of other existing comprehensive care plans. Complete all Assessments and Support Plans according to prescribed timelines and specifications.Act as a liaison between members, community agencies, and families to coordinate treatment, including employment and education entities, Care Coordination entities, Emergency Service Providers, outpatient clinics, and others.
