Open Sky Community Services in Worcester, MA seeks an Enrollment and Care Coordination Specialist to screen, enroll, and support members in CSP-HI, housing and nutrition programs, with data entry into the electronic health record system. The role includes coordinating with housing specialists and ensuring service orientations and consent are completed.

The ideal candidate will have at least two years of housing/behavioral health experience, strong communication, and a valid driver’s license.

Qualifications

  • High School Diploma, GED or equivalent, required.
  • Minimum of two years’ experience with housing and behavioral health population, required.
  • Valid driver's license and acceptable driving history, required.
  • Experience with electronic health records, billing and/or referral management strongly preferred.

Responsibilities

  • Screen individuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility.
  • Complete enrollment including orientation to services, consents, and data entry into the EHR.
  • Provide training, monitoring, feedback and corrective action to housing specialists.
  • Assign and track housing specialists caseloads in collaboration with the team lead.
  • Outreach to community providers to develop relationships and referral pathways.
  • Verify insurance eligibility and monitor insurance eligibility for each member; obtain authorizations and track expirations.
  • Participate in live review meetings with insurance company representatives for each member.

Job Description:The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection.The Enrollment and Care Coordination Specialist will screen individuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboration with the team lead, as needed.Other Key Responsibilities:Outreach to community providers to develop relationships and develop referral pathways.Verify insurance prior to enrollment and monitor insurance eligibility for each member.Obtain authorizations from the insurance provider for each member.
