Job summary

Lakes Regional Community Center in Texas seeks an Enhanced Community Coordinator to provide intensified service coordination for individuals with intellectual and developmental disabilities transitioning from facilities. Qualifications

  • Or a high school diploma with two years paid case management experience and related eligibility.

Responsibilities

  • Assess and verify service coordination needs and develop service plans.
  • Refer and monitor service provision; advocate for consumers.
  • Conduct afterhours crisis services and onsite visits for post-move monitoring.
  • Review providers’ records; monitor progress and document outcomes.
  • Participate in planning meetings; maintain monthly face‑to‑face contact with consumers and frequent contact with families and providers.
  • Submit CLDP reports to HHSC for individuals transitioning from SSLC; travel locally as required.

Job description

Enhanced Community Coordinator

The Enhanced Community Coordinator (ECC) provides enhanced Service Coordination to persons with Intellectual and Developmental Disabilities that have transitioned from or been diverted from Nursing Facilities or State Supported Living Centers (SSLC). The ECC provides intensive and flexible support to individuals to ensure success in the community setting, including arranging for support needed to prevent and manage a crisis, and accessing community-based services and resources.

Responsibilities include assessment and verification of service coordination needs; service planning; referral; monitoring or service provision; advocacy for consumers; afterhours crisis services; conducting onsite visits of service delivery sites for post‑move monitoring; reviewing Program Providers’ records and implementation plans; intensive monitoring, including inquiries about recent hospitalization, ER visits, and crises; and documentation of progress. Responsibilities involve participating in service planning team meetings; monthly face‑to‑face contact with consumers, frequent contact with family members and service providers; and submitting reports to HHSC as scheduled or upon request, including CLDP reports to SSLC and HHSC for individuals who transitioned from SSLC. Extensive travel within the local service area is required. To accommodate consumer and family needs, must be able to work a flexible schedule, including afterhours, weekends, and holidays.

Minimum Qualifications
  • Bachelor’s or advanced degree from an accredited college or university with a major in a qualified human services field to include but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
  • Or a high school diploma or GED and: (1) two years of paid experience as a case manager in a state or federally funded Parent Case Management Program, or have graduated from Partners in Policy Making; and (2) personal experience as an immediate family member of an individual with intellectual and developmental disabilities.
  • Must have proficient computer skills to include MS Word, Excel and Outlook.
  • Must have valid driver’s license and an acceptable driving record, as well as personal automobile liability insurance as required by the state of Texas.
Preferred Qualifications
  • Two or more years of work experience as a Service Coordinator, case manager, or related position serving individuals with Intellectual and Developmental Disabilities.
  • Bilingual skills (English/Spanish).
