Job summary

The Service Coordinator position at Bluebonnet Trails Community Services supports children and adults with developmental disabilities using Person-Centered Thinking. You assess needs, develop service plans, facilitate linkages, and monitor effectiveness while advocating for individuals and ensuring services meet Medicaid/HHSC/MCO guidelines.

Travel within the service area is required; positions require flexible hours and driving.

Qualifications

  • Associate degree or higher in social, behavioral, or human service field.
  • High school diploma with two years of paid or unpaid experience with individuals with intellectual or developmental disabilities.
  • Must meet driving requirements and maintain approved driving status.

Job description

The Service Coordinator position provides services to children and adults with developmental disabilities based on the Person-Centered Thinking model through assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports and the delivery of advocacy services. The ability to work with families and sensitivity to various agency and interagency personnel to accomplish objectives. The ability to work with families and sensitivity to various cultural differences is critical. The purpose of the position is to empower individuals to achieve life goals and to live effectively within their families and/or communities. The Service Coordinators will monitor services which will be provided to individuals in accordance with standards and billing guidelines established by Medicaid, HHSC, MCOs and BTCS, as they apply to individual cases. This includes but is not limited to advocating and linking individuals interested in job readiness and/or employment activities through the Employment First project. The service coordinator will participate in interagency planning and CRCG meetings to assist in bridging gaps in services and avoid duplication in services in the communities where individuals receive services.

Job responsibilities require traveling within the service area and flexible hours and schedule. Essential duties include completion of comprehensive assessments of individual needs and desires, service planning, referral and linkage, monitoring and advocating on behalf of individuals served, documentation of services provided and interaction with the individual/family served.

Hourly Salary Range: $

20.46 - $2 .21

Differential Detail
  • .75/hour, Bilingual
  • $1.00/hour, Human Services Bachelor's Degree
  • $1.00/hour, Associate or Advanced Licensure
  • Up to $4.00/hour, tenure with BTCS

Minimum Qualifications

An associate degree or higher is required with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice OR A high school diploma or certificate recognized by the state of Texas as the equivalent of a high school diploma, with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities. Must meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment.

Preferred Qualifications

At least one year of experience working with individuals with intellectual/developmental disabilities. Bachelors Degree. Demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting. Demonstrated computer and documentation skills. Knowledge of appropriate community resources. Bilingual (English/Spanish/ASL). Experience with Medicaid programs.
