Job summary

Qualifications

  • Knowledge of Medicaid programs.
  • Computer and documentation skills.
  • Knowledge of appropriate community resources.

Job description

Overview

The Service Coordinator position provides services to children and adults with developmental disabilities based on the Person‑Centered Thinking model. Responsibilities include the assessment of needs, development of service plans, facilitation of service linkages, monitoring of service effectiveness, provision of crisis intervention/prevention supports, and delivery of advocacy services. The Coordinator works with families, agency personnel, and members of the community to empower individuals to achieve life goals and live effectively within their families and communities.

Job Responsibilities

Key duties are:

  • Complete comprehensive assessments of individual needs and desires.
  • Develop service plans and facilitate referrals and linkages.
  • Monitor and advocate on behalf of clients, ensuring services meet standards and Medicaid billing guidelines.
  • Document services provided and maintain accurate client records.
  • Participate in interagency planning and CRCG meetings to bridge service gaps.
  • Assist in job readiness and employment activities through the Employment First project.
  • Travel within the service area and work flexible hours and schedule.
  • Use initiative, independent judgment, and professional intervention to address client needs.
Salary

Hourly Salary Range: $20.46 – $27.21

  • .75/hour, Bilingual
  • $1.00/hour, Human Services Bachelor’s Degree
  • $1.00/hour, Associate or Advanced Licensure
  • Up to $4.00/hour, tenure with BTCS

Credentials

Minimum Qualifications

  • Associate degree or higher with a major in social, behavioral, or human service fields, including but not limited to psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.
  • High school diploma or certificate recognized by the state of Texas as equivalent to a high school diploma, with two years of paid or unpaid experience working with individuals with intellectual or developmental disabilities.
  • Must meet Bluebonnet Trails Community Services driving requirements and maintain approved driving status throughout employment.

Preferred Qualifications

  • At least one year of experience working with individuals with intellectual/developmental disabilities.
  • Bachelor’s Degree.
  • Demonstrated ability to use good clinical judgment, collaborate with others, and work effectively in a fast‑paced setting.
  • Computer and documentation skills.
  • Knowledge of appropriate community resources.
  • Bilingual (English/Spanish/ASL).
  • Experience with Medicaid programs.
