Chartiers Center is seeking an Enhanced Blended Service Coordinator to join the Janus Program. This full-time role involves coordinating benefits, treatment and support services while independently assessing needs and developing individualized care plans.

Expect regular participant meetings and involvement in rotating on-call coverage to support program participants. Minimum qualifications include a Bachelor’s degree in human services with 3 years of mental health experience or a Master’s in

• Coordinate and oversee the delivery of benefits, treatment and support services to participants.
• Independently assess participant needs and develop individualized plans that provide the intensity and frequency necessary.
• Meet with participants on a frequent basis to provide needed interventions and monitor progress.
• Participate in rotating on-call services to support the Janus program participants.

Chartiers Center is a private, non-profit corporation funded by state, county, third party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program. The Janus Program is a new and exciting model of care that uniquely assists participants to liven on their own and learn desired skills. The Enhanced Case Manager is a member of the multi-disciplinary team that works closely with program participants.Although we embrace a "whatever it takes" approachthe Enhanced case manager provides personalized and specialized Service Coordination to participants of the Janus Program.

As an Enhanced Blended Service Coordinator with Chartiers Centerthecore job dutiesinclude:
