Chartiers Center is a private, non-profit corporation funded by the state, county, 3rd party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach and housing program.

Position Overview

As a Housing/Re-entry Coordinator you are responsible for locating and identifying housing opportunities, providing referrals, linkages and support related to housing, benefits, and mental health/physical health care to individuals and their families experiencing homelessness who also have mental health issues and justice system involvement.

Responsibilities

• Collaborate with inpatient unit, emergency departments, jails and legal system to facilitate coordination of care.
• Serve as a clinical monitor and bridge to implement strategies supporting overall wellness, particularly in securing and retaining permanent housing.
• Provide services in a variety of non-traditional settings, including the person/family’s home, on the street, and in clinics.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, Nursing or related field and at least one year experience in justice‑related services and experience working with chronically homeless individuals and families to gain permanent housing.
• OR Associate's degree in Social Work, Psychology, Counseling or related field and two years’ experience in justice‑related services and experience working with chronically homeless individuals and families to gain permanent housing.
