Job summary

Chartiers Center in Dormont, PA, is seeking a Direct Support Specialist to provide coaching and assistance to individuals with intellectual disabilities in a day program setting. Responsibilities include conducting support activities, administering assessments, and developing personalized plans.

Ideal candidates will have a Bachelor's degree in Human Services with at least 3 years of relevant experience, or an Associate's degree with four years of IDD work experience. Chartiers Center is a private, non‑profit corporation funded by state, county, third‑party insurances and private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitative services, crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program.

As a Direct Support Specialist with Chartiers Center, you will be providing coaching and assistance to people with intellectual disabilities in a day program setting.

Responsibilities

• Conduct instructive and support activities for individuals designed to meet the individual’s plans outlined in an interdisciplinary meeting and the support plan.
• Administer assessments and evaluation instruments to assist in identifying abilities.
• Develop and implement individualized plans, following supervisory approval, based on person‑centered plan of needs, wants, likes and dislikes of individuals.
• Prepare monthly progress reports on a timely basis using pre‑designated schedule; document thorough and concise data on how to relate to self and others.
• Document in electronic health record according to regulated compliance timelines.
