Job summary

Chartiers Center, Inc. seeks a Mobile Enhanced Psychiatric Rehabilitation Worker to provide in-home and community-based services.

You will meet weekly with clients to teach independent living, social, and job-search skills, while connecting them with living resources and social opportunities. Responsibilities include documenting meetings, maintaining a heavy face-to-face schedule, and rotating on-call duties.

Qualifications

• Bachelor’s Degree with two years of mental health experience.
• Valid driver’s license.

Responsibilities

• Meet weekly with a caseload of clients in their home or community to teach them needed skills and connect them with supports.
• Maintain a weekly schedule with in-person client meetings.
• Thoroughly document client meetings reflecting interventions and client responses.
• Provide on-call on a rotating basis.
• Available to work a flexible schedule.
• Complete Comprehensive Assessments and Individual Rehabilitation Plans for assigned caseload.

As aMobileEnhancedPsychiatric Rehabilitation Worker with Chartiers Center, you will work in the home and community with our program participants to help them to live full and productive lives. Job Duties include:

• Meet weekly with a caseload of clients in their home or community to teach them needed skills such as independent living skills, social skills, and job search skills, and help connect them with supports such as needed living resources and social opportunities
• Maintain a weekly schedule where the majority of your work time is spent in face-to-face client meetings
• Thoroughly document client meetings to reflect work completed and interventions delivered, as well as client’s response to service and any client concerns
• Provide on-call on a rotating basis.
• Available to work a flexible schedule
• SpecialistOnly- Complete Comprehensive Assessments and Individual Rehabilitation Plans for assigned caseload.
