Job summary

Chartiers Center is a private, non-profit organization providing extensive community-based services including intake, therapy, crisis intervention, housing, and support for adults with intellectual disabilities and other needs. The Mobile Case Manager role involves community-based support to individuals facing mental health challenges and coordination of benefits and services.

• Assess individuals’ needs and develop individualized service plans.
• Maintain regular communication with clients to monitor progress and address concerns.
• Collaborate with other service providers and staff to ensure seamless delivery of services.
• Maintain strong community relationships with contacts for education, employment, housing, and natural supports.
• Maintain accurate records of interactions, service plans, and progress reports.

Job description

Chartiers Centeris a private, non-profit corporation funded bystate, county, third partyinsurancesand private funds. Services include intake, referral, outpatient therapy, psychiatric assessment, service coordination, substance abuse treatment, training and social rehabilitativeservices,crisis intervention, partial hospitalization, community outreach, day, and recreational services for adults with intellectual disabilities, homeless outreach, and housing program.

As a Mobile Case Manager with Chartiers Center,you wouldbe working in the community to provide support and services to individuals with mental health challenges.

Job duties include:

• Coordinate and oversee the delivery of case management services to ensure that the individuals are connected tovarious benefitsand services such as SNAP, CAP, LIHEAP, LIHWAP, SOAR, OP, and MPR.


• Assessindividual’sneeds and develop individualized service plans to meet those needs.


• Maintain regular communication with clients tomonitorprogress and address any concerns.


• Collaborate with other service providers and/or administrative staff to ensure seamless delivery of services.


• Maintain strong community relationships with contacts for education, employment,housingand naturalsupports.


• Maintainaccuraterecords of interactions, service plans, and progress reports.

Minimum Qualifications:
