Job summary

Chartiers Center in Bridgeville, PA is seeking a Mental Health Residential Coordinator to support adults facing behavioral health challenges within our Residential Treatment Program. You will help residents access services, monitor progress, and maintain a safe, therapeutic environment.

Duties include developing, implementing, and documenting Residential Service Plans; delivering daily living training; recording medication administration and observations; applying treatment team recommendations;

Qualifications

• Bachelor's degree in Human Service field.
• Three years’ experience in a behavioral health setting.
• Valid PA Driver’s License.

Responsibilities

• Develop, implement, monitor, and document Residential Service Plans for an assigned caseload.
• Complete skill training in daily living through hand-over-hand assistance and observation.
• Document resident self-administration of medication, counts, and follow-up.
• Implement treatment team recommendations to ensure program consistency.
• Maintain resident records per policies and regulations.
• Assist the Director with daily operations and staff scheduling.
• Respond to emergencies/incidents per policies.

As a Mental HealthResidentialCoordinatorwith Chartiers Center in our Residential TreatmentProgramyouareresponsible forprovidingassistancetoadults facing mental health challenges.

Job duties include:

• Develop, implement, monitor, and document Residential Service Plans for an assigned caseload of residents.
• Completing necessary skill training in the areas of daily living through hand over handassistance, direct verbal and written presentation of information and observation, as assigned.
• Accurately complete all assignedobservation,intervention, documentation of resident self-administration of medication, medication counts and necessary follow-upin accordance withProgram policies and procedures.
• Accurately implement recommendations of the treatment team to ensure consistency within the program.
• Maintain resident records in accordancetopolicies,proceduresand regulations.
• Complete laundry,cleaningand shopping asrequired.
• Effectively respond to,reportand document all emergencies/crises/incidents in accordancetorelevant policies,proceduresand regulations.
• Assistthe Director of the Residential Treatment Program to effectively oversee the daily operations of the program and inscheduling ofstaff /overall management of the schedule.

Minimum Qualifications
