Job summary

Chartiers Center is seeking a Blended Service Coordinator to support adults with serious mental illness and co-occurring conditions in community settings. You will build relationships, advocate for individuals, and help them achieve stability and independence.

The role involves coordinating services across housing, healthcare, and behavioral health providers, with travel throughout the community and an expectation of timely documentation and collaboration.

Qualifications

• Associate’s degree in human services and 3+ years of mental health experience.
• Bachelor’s degree in human services and 2 years of mental health experience OR master’s degree in human services with one year of experience.
• Valid driver’s license required.

Responsibilities

• Meet with individuals in homes and community settings.
• Conduct assessments and identify strengths, needs, and goals.
• Develop, implement and monitor individualized service plans.
• Connect individuals with housing, benefits, healthcare, behavioral health treatment, employment and community resources.
• Coordinate services among healthcare providers, therapists, family members and community agencies.
• Maintain regular communication with individuals to monitor progress and identify emerging needs.
• Advocate on behalf of individuals across multiple systems.
• Maintain timely and accurate documentation.
• Participate in an on‑call rotation, typically no more than once per month.

Role Overview

As a Blended Service Coordinator with Chartiers Center, you would provide support and service coordination to adults living with serious mental illness and co‑occurring conditions, including substance use disorders, physical health concerns and intellectual disabilities. This highly rewarding community‑based role involves building meaningful relationships, advocating for individuals and helping them achieve greater stability, independence, and recovery.
