Job summary

Merakey in Sharon Hill, PA, seeks an Engagement Specialist to join our Delaware County program. This full-time role coordinates care with hospitals and external agencies to meet social determinants of health needs.

You will develop relationships with inpatient units, emergency departments, residential settings, and community resources, promote Merakey programs at events, and help consumers complete referral information to link them to services.

Qualifications

• Bachelor's degree in chemical dependency, sociology, social welfare, psychology, nursing, or related field, or 18 college credits in related social sciences.
• Clear criminal background check and Child Abuse clearance.
• Basic computer skills.
• Clean, valid driver’s license and transportation for travel within Delaware County.

Responsibilities

• Coordinate care with hospitals and external agencies to meet social determinants of health needs.

This role is full-time, Monday - Friday 9:00a - 5:00p.

Earn $24.68/hr.!

The Engagement Specialist will work closely with hospitals and other external agencies to coordinate care and resources for individuals in Merakey programs. This position will focus on developing relationships with hospital inpatients and emergency departments, residential settings, and treatment facilities, in addition to external resources to meet social determinants of health needs of individuals in services. This individual will attend community, internal, and stakeholder events to promote the programs and generate referrals. This individual will also work with consumers individually to assist in completing and collecting referral information to link them to a Merakey program or other resources within the community.

Requirements

The ideal candidate will possess the following qualifications:

• A bachelor’s degree in chemical dependency, sociology, social welfare, psychology, nursing, or a related field; OR a successful completion of 18 college credits in sociology, social welfare, psychology, criminal justice, or other related social sciences
• A clear criminal background check and Child Abuse clearance
• Basic computer skills
• A clean, valid driver’s license and personal transportation for travel within Delaware County
