Merakey is seeking an Engagement Specialist to join our program in Delaware County, PA. This full-time role focuses on coordinating care and building relationships with hospitals, EDs, residential settings, and community resources to meet social determinants of health needs.

This role is full-time, Monday - Friday 9:00a - 5:00p.

Earn $24.68/hr.!

Position Details

The Engagement Specialist will work closely with hospitals and other external agencies to coordinate care and resources for individuals in Merakey programs. This position will focus on developing relationships with hospital inpatients and emergency departments, residential settings, and treatment facilities, in addition to external resources to meet social determinants of health needs of individuals in services. This individual will attend community, internal, and stakeholder events to promote the programs and generate referrals. This individual will also work with consumers individually to assist in completing and collecting referral information to link them to a Merakey program or other resources within the community.

The ideal candidate will possess the following qualifications:
