Job summary

Jobtailor is seeking an LPN to join our clinical team in Illinois. You will be responsible for rooming patients, collecting assessment data, maintaining charts, and assisting with examinations and procedures.

The role requires licensure as an LPN, CPR/BLS certification within 90 days, and preferred clinical experience. You will perform injections, document orders, coordinate referrals, and participate in QA projects while providing patient education.

Qualifications

• Graduate of an accredited LPN Program.
• Illinois: Ladder Option Approved Designated RN Program completed.
• Experience in clinical setting preferred.
• Licensed as an LPN in the State of Practice.
• CPR or BLS certification within 90 days.
• Ability to perform duties within LPN scope of practice, licensure, policy and State Practice Act.

Responsibilities

• Room patients, collect assessment data, and prepare patients according to specific needs.
• Collect patient data and document in medical records
• Prepare and maintain patient charts
• Assist with examinations, treatments, and clinic procedures
• Administer injections and medications as ordered and directed
• Review physician orders, lab requests, and follow-up needs with patients
• Assist with laboratory procedures, screening tests, and QI projects
• Answer telephone inquiries or direct callers to appropriate personnel for triage
• Provide ordered patient treatments and document according to standards of care
• Coordinate patient referrals through insurance and other physician offices
• Observe patients and report adverse reactions to medication or treatment
• Assist with clinic projects, nursing initiatives, surveys, peer review, and quality assurance
• Reinforce patient teaching and provide basic healthcare instruction
• Perform EKGs, simple lab tests, and venipuncture
• Order and maintain supplies

Requirements

• Graduate of an accredited LPN Program OR Wisconsin: Have completed the portion of the RN Program needed to obtain a certificate of completion in practical nursing.
• Illinois: Have completed the designated curriculum of a Ladder Option Approved Designated RN Program.
• Experience in clinical setting preferred.
• Licensed as an LPN in the State of Practice
• Approved CPR or BLS certification within 90 days
• Ability to perform duties within LPN scope of practice, licensure, policy and State Practice Act
